Current Student-Athletes

Requirements for Athletic Participation

All incoming first-year SUNY Plattsburgh athletes, including transfers, must complete this form.

           Step 1: Have a completed physical signed by your doctor

Documentation must be on file at Student Health Center prior to your arrival on campus. Please make sure there is a statement from the doctor on the physical that states if you are or are not fit to participate in sports.

Step 2: Complete the online medical questionnaire

            Please log on to http://plattsburgh2.atsusers.com to complete this form as required by our medical records policies. If you are using the questionnaire for the first time,             type the word "new" for both the user ID and password. You must reset your user ID and password in the personal information section when you begin the form process.             Your personal user ID and password are case-sensitive. All information is confidential and only the Athletic Training staff will have access to the forms. This form MUST be             completed prior to pre-participation screening.

Step 3: Provide a current copy of your current insurance card (front and back)

If you do not have insurance, or if you have purchased international insurance, please inform the athletic training staff as soon as possible. If you are currently injured or have seen a Doctor or any other healthcare professional in the off-season, please submit:

  • All doctors/ healthcare professional notes and reports including x-ray reports, MRI reports
  • Follow-up care notes
  • Written return to play clearance

If you have had surgery or other medical work-up (injuries or illnesses that have prevented you from participating in your sport) performed within the last 3 years, please submit all paperwork from your physician stating what the work-up was for and if you have been released for sports.

If you sustain an injury, the first thing you need to do is report the injury to one of the Athletic Trainers.

All Doctor visits and medical testing will be referred through the Athletic Training staff.
Failure to comply will resolve SUNY Plattsburgh and its affiliates of any financial responsible.

SUNY Plattsburgh has a policy requiring all students to carry a form of health insurance while on
campus at SUNY Plattsburgh. Athletes at SUNY Plattsburgh are not exempt from having their own
insurance. It is the policy of the Intercollegiate Athletic Department that all Student-Athletes provide
proof of insurance and provide a copy of the insurance card (front and back) to the Athletic Training
Office.

SUNY Plattsburgh's Intercollegiate Athletic Department DOES NOT have a secondary policy for
athletic-related injuries. Due to this policy, it is REQUIRED by both the NCAA and SUNY Plattsburgh
that proof of insurance is provided prior to an authorized practice or competition for 2023-2024
academic year. The NCAA has a catastrophic injury insurance policy on all athletes for any medical
expenses above $90,000. Any primary insurance company must be able to provide up to $90,000 in injury
coverage for SUNY Plattsburgh and its Student-Athletes to be in compliance with the NCAA catastrophic
injury policy.

Due to this, it is the responsibility of the Student-Athlete and/or the family to pay any finances
due (including co-pays and deductibles) regarding injuries sustained during an athletic event as a result of
participating in SUNY Plattsburgh Intercollegiate Athletic Department.
